WHAT ARE THE CAUSES OF HIP PAIN AND BURSITIS?
Hip bursitis and tendinitis is an extremely common cause of musculoskeletal pain and disability, which can become chronic. Hip bursitis is commonly caused from a combination of overuse, poor hip mobility/strength, altered biomechanics and trauma. This often leads to significant localised pain, poor function and on going disability. Most cases of hip pain and bursitis are caused by a combination of overuse, repetition and injury. Severity can vary. Often bursitis and gluteal tendinitis can be linked.

SYMPTOMS OF HIP BURSITIS

DIFFERENCES BETWEEN ACUTE & CHRONIC HIP PAIN
-
ACUTE PAIN is often due to direct trauma or injuries such as ligament tears, or sprains.
-
CHRONIC PAIN may be a results from progressive wear and tear, repetitive strain, or degenerative conditions like osteoarthritis.
It is very important to be aware of more sinister causes of hip bursitis, like inflammatory arthritis. Other important symptoms to be aware of are multiple joint pain/swelling, fever, altered sensation or tingling down the leg, ongoing cramps in the legs, fatigue, weight loss and night pain.
COMMON HIP CONDITIONS WE SEE & TREAT
At our Birmingham Hip Pain Clinic, we specialise in diagnosing and treating a wide range of hip issues using a wide range of treatments, including regenerative and nonsurgical therapies. Below is a list of some of the most common issues we address in our clinics across Birmingham and Warwickshire.
Hip osteoarthritis occurs when the cartilage in the hip gradually deteriorates, resulting in pain, stiffness, limited mobility, and challenges with walking everyday tasks. At Dynamic Regenerative Medicine, we emphasise joint-preserving therapies, such as image-guided injection therapy, aimed at reducing inflammation, enhancing joint lubrication, promoting cartilage health, and potentially delaying or preventing the need for surgery.

Gluteal tendinopathy is a frequent source of lateral hip pain, typically experienced on the outer of the hip and aggravated by activities such as walking, climbing stairs, or lying on the affected side. At Dynamic Regenerative Medicine, we prioritise joint preservation through conservative management that includes manual and physical therapies (osteopathy and physiotherapy) alongside advanced injection therapy. This approach aims to alleviate tendon-related and inflammation, enabling patients to participate more effectively in targeted designed to restore strength, load tolerance, and hip stability.
Hip bursitis happens when the bursae surrounding the hip joint become inflamed, typically due to repetitive strain, changes in patterns, or excessive tendon stress. Symptoms often include localised pain and tenderness on the outer hip area. Imaged injection therapy can effectively relieve pain by reducing inflammation, facilitating better movement, and supporting efforts.

Hip Impingement (Femoroacetabular Impingement – FAI)
Hip impingement occurs when abnormal contact between the ball and socket of the hip joint restricts movement and irritates surrounding tissues. This can cause groin pain, stiffness, and discomfort during prolonged sitting or sporting activities. Injection therapy in combination with conservative management may be used to reduce joint inflammation and pain, supporting improved mobility and more effective rehabilitation.
Labral Irritation and Hip Joint Pain
Hip impingement occurs when abnormal contact between the ball and socket of the hip joint restricts movement and irritates surrounding tissues. This can cause groin pain, stiffness, and discomfort during prolonged sitting or sporting activities. Injection therapy in combination with conservative management may be used to reduce joint inflammation and pain, supporting improved mobility and more effective rehabilitation.

The Aim of Injection Therapy in Hip Pain Management
At Dynamic Regenerative Medicine, injection therapy is an important part of pain treatment approach. Image-guided injections allow us to precisely target the source of pain, reduce inflammation and support tissue function. When combined with expert rehabilitation and movement correction, these treatments can significantly improve pain, mobility, and long-term joint health.
Why you may Choose Injection Therapy for Hip Pain?
Injection therapy enables us to target the source of pain in the soft tissues surrounding the hip directly. With the help of image-guided precision, we can decrease inflammation enhance tissue healing, and foster an ideal healing environment. This approach results in quicker pain relief, enhanced mobility, and greater participation in rehabilitation.
Supporting Cartilage Health and Joint Preservation
For numerous hip conditions, especially osteoarthritis and cartilage, the aim extends beyond mere pain relief to include long joint preservation. Our injection therapies are tailored to promote cartilage, minimise wear, and slow down degenerative changes in the hip, assisting patients in preserving function and potentially postponing or preventing need for surgery.
Combining Injections with Expert Rehabilitation
Injection therapy is most effective when combined with a structured rehabilitation programme. By reducing pain and inflammation first, patients are able to move more freely, strengthen key muscle groups, and correct biomechanical faults. This integrated approach leads to more durable outcomes and reduces the risk of recurrence.
WHAT ARE THE TREATMENT OPTIONS FOR HIP BURSITIS & TENDINITIS?
There are many different treatment options that can provide assistance and treat hip pain and bursitis. It is the aim to reduce pain and inflammation at the same time of working on the predisposing factors that may be contributing towards to the issue. Treatments may include:

-
Physiotherapy (physical therapy)
-
Rehabilitation
-

Hip bursitis and tendinitis can be triggered by many different reasons. Ultimately, we want to reduce pain, improve function and prevent further long term damage. At Dynamic Regenerative Medicine, lateral hip pain and bursitis is an extremely common issue we see and treat every day. We always stress the importance of treating hip bursitis early, to treat and prevent the likelihood of chronic damage to the local soft tissues. When hip bursitis becomes chronic it can often be related to damage to the gluteal tendons which can be more challenging to treat. That being said, we treat all cases of hip bursitis, most of which are dealt with successfully.
In the initial stages we like to enforce conservative treatment and management of hip pain and tendinitis. But in many cases, extra resources may be encouraged. The Dynamic Clinic have a very good reputation in the treatment and delivery of high quality regenerative treatments for hip bursitis and tendinitis. The most popular by far being P-PRP injection treatment, in which patients travel from all different regions of the UK to see us for advice and treatment (further information can be found on the ORTHOBIOLOGICS link below). If patients are showing signs of advanced injury or disease then we may discuss the option for scans to get diagnostic clarity of what it is that may be causing the issue and the severity.
If a cause of your hip pain is suspected to be of an inflammatory or rheumatological origin, then you will be asked to provide a blood test and lab panel to check for raised inflammatory markers - ESR, CRP and TNFa, and maybe immunological makers.
Specialist Imaging or Private MRI Scans
At Dynamic Regenerative Medicine, our hip pain clinics in Birmingham and Warwickshire can provide ultrasound imaging to look for signs of tissue damage and inflammation. However, on occasions we may require a more in-depth analysis. If a private MRI scan is considered, we will put in a local referral to our Imaging Centre in Solihull. You will then be contacted and booked in within the next few days. Following your scan, the report will be completed by our colleague Dr David Beale (Consultant Musculoskeletal Radiologist). This report will then come back to us and will we then interpret the images and discuss with you then best course of treatment for your complaint.

It should be noted that advanced cases of hip osteoarthritis are extremely difficult to manage conservatively and surgical opinion may well be advised. In these cases, we may refer to our trusted colleagues Mr Shiv Jain (Consultant Orthopaedic Surgeon) for opinion on hip surgery or total replacement.
INJECTION TREATMENT FOR HIP BURISTIS AND TENDINITIS
Sometimes hip injuries to soft tissue can turn into chronic pain, this especially common with hip bursitis and gluteal tendinitis. In these cases, sometimes the body needs a push to aid healing and repair. Injection therapy for lateral hip pain, injury and bursitis can also be commonly used to treat stubborn cases and can deliver great success in many cases. There are many different forms of injection therapies but the most common are steroid injections, platelet rich plasma (PRP) injections and hyaluronic acid.

Individuals who have been complaining of lateral hip pain who have not responded effectively to standard treatment and management may be advised to consider a cortisone injection to reduce pain and inflammation. However, it is very important to understand that steroid injections (if successful) are often only short-lived and may have an overall degenerative effect on the local tissue if repeated. Although they can be good for inflammation, cortisone injections are not routinely used for on going pain management due to weakening of tissue. This can put tendons at an increased risk for rupture (tear), if repeated too often.
Hugely popular, PRP treatment is a safe natural treatment commonly used in many different cases of musculoskeletal pain and injury, that uses platelets from the patient’s blood to promote and facilitate healing and repair in damaged tissue. Increased scientific literature from evidence-based clinical trials are increasingly showing how PRP injection treatment is safe and effective for the treatment of hip bursitis and hip gluteal tendinitis, also showing improved benefits over cortisone, particularly for long term progress.
