What are the causes of back pain?

Back pain is one of the most common causes of pain and disability worldwide, and can range from short episodes of localised discomfort through to extremely significant levels of pain. Most cases of back pain, especially low back pain, are triggered by a combination of, poor posture, overuse, repetitive strain, injury (or trauma), and inflammation to the structures associated with the spine. Severity can vary, but below are some examples of common causes of back pain.

What are the common types of back pain?

  • Soft tissue injury to muscles, tendons, ligaments

  • Spinal facet joints inflammation

  • Injury or irritation to spinal nerves that pass through the spine

  • Degenerative changes to spinal joints

  • Compression or reduced blood flow of the spinal nerves or spinal cord

  • Disc injury such as bulging or herniated disc that can compress nerve roots (sciatica)

  • Degenerative changes to disc

  • Spinal arthritis

  • Osteoporosis

ACUTE VS CHRONIC BACK PAIN

  • ACUTE PAIN is often due to direct trauma or injuries such as ligament tears, or disc injuries.

  • CHRONIC PAIN  may be a results from progressive wear and tear, repetitive strain, or degenerative conditions like osteoarthritis.

It is also very important to be aware of more sinister causes of back pain, like inflammatory arthritis, or tumours. Other important symptoms to be aware of are prolonged morning joint stiffness, fever, altered sensation or tingling down both legs and/or around the genitals/anus, ongoing cramps in the legs, abdominal pains,  bowel and bladder incontinence, fatigue, weight loss and night pain.

WHAT ARE THE COMMON TYPES OF BACK PAIN WE TREAT?

At our Birmingham Back Pain Clinic, we specialise in diagnosing and treating a wide range of back complaints using a wide range of treatments, including osteopathy and physiotherapy. Below is a list of some of the most common issues we address in our clinics across Birmingham and Warwickshire.

Disc injuries occur when the soft cushioning discs between the vertebrae become damaged or displaced. This can lead to localised pain, stiffness, or radiating nerve symptoms, including tingling or weakness in the legs. At our Birmingham & Warwickshire clinics, we offer various treatments, including and guided rehabilitation, to relieve pain and support natural disc healing.

Facet Joint Inflammation

Facet joints are small joints at the back of the spine that help guide movement. Inflammation or wear-and-tear of these joints can cause persistent back pain, stiffness, and discomfort during bending or twisting. Our specialists provide targeted physiotherapy, joint injections, and personalised rehabilitation programs to improve mobility and reduce pain.

Ligament Injuries

Spinal ligaments connect the vertebrae and provide stability. Sprains or tears of these ligaments often result from trauma or overuse, causing sharp or ongoing pain. We focus on combining regenerative treatments, manual therapy, and structured exercise programs to restore spinal stability and function.

Sciatica arises when the sciatic nerve is irritated or compressed, commonly by a herniated disc or spinal stenosis. Patients often experience pain, numbness, or tingling radiating from the lower back down the leg. At Dynamic Regenerative Medicine, our nerve-focused therapies, including and rehabilitation, help reduce nerve pressure and improve mobility.

Muscle Strains and Spasms

Back muscles can become strained or overworked due to sudden movements, poor posture, or repetitive activities. This can cause pain, stiffness, and muscle spasms. Our Birmingham clinic provides targeted physiotherapy, strengthening exercises, and regenerative support to relieve pain and prevent recurrence.

Spinal Stenosis

Spinal stenosis is the narrowing of the spinal canal, which can compress the spinal cord or nerves. Symptoms often include back pain, leg numbness, and difficulty walking. We provide treatments aimed at reducing inflammation, improving spinal mobility, and relieving nerve compression.

The sacroiliac joint connects the spine to the pelvis, and dysfunction can lead to lower back or buttock pain. Our team offers accurate diagnosis and interventions, including joint injections and physiotherapy or manual therapy, to restore alignment and reduce discomfort.

Osteoarthritis of the Spine

Degeneration of the spinal joints and discs can cause chronic back pain, stiffness, and reduced range of motion. At Dynamic Regenerative Medicine, we combine manual treatments like osteopathy, and exercise-based rehabilitation to manage symptoms and enhance daily function.

Back pain and injury is one of the biggest complaints we see here at the Dynamic Clinic, seeing multiple times every day. Fortunately, the vast majority of back pain cases we see are dealt with and treated successfully with conservative treatment modalities. We are very proud of the fact that less that 5% of our back pain patients are considered for invasive therapeutic treatment. If patients do not respond adequately to conservative treatment or the problem is getting worse, then consideration for injection therapy may be discussed. If patients are showing signs of neurological deficit then we may discuss the option for an MRI scanning, to get diagnostic clarity of what it is that may be causing the issue.

If an inflammatory or rheumatological cause of your back pain is suspected, then you will be asked to provide a lab panel and blood test to check for raised inflammatory markers - ESR, CRP and TNFa, and maybe immunological makers.

Referral for Specialist Imaging or Private MRI Scans

At the Dynamic Clinic we can provide ultrasound imaging to assess for signs of tissue damage and inflammation. But, occasionally, we may require a more in depth analysis. When a private MRI is considered, we will put in a local referral to our close Imaging Centre. You will then be contacted and booked in within 5-10 days. Following this scan, the report will be completed by our colleague Dr David Beale (Consultant Neuro-radiologist). This report will then come back to us and will we then discuss with you then best course of action.

In some cases we may refer to our trusted colleagues Dr Abir Dodger (Consultant Anaesthetist) or Mr Spencer Harland (Consultant Spinal Neurosurgeon) for opinion on spinal denervation or for surgical option on micro-discectomy.
