What are the causes of frozen shoulder?

Frozen shoulder is very common and can be a long winded process and very challenging to treat. The causes of frozen shoulder are a bit of a mystery, but it is thought to be largely due to trauma, immunological compromise, and autoimmune correlation, like the connection with diabetes.  The condition is clinically known as adhesive capsulitis, which mean inflammation, thickening and sticking/freezing of the shoulder joint capsule. This often leads to signifiant pain and very poor mobility. Frozen shoulder is often self-remitting, but known to have a very long process of 6-18 months. During this process often many patients seek pain relief and improved recovery.

Symptoms of frozen shoulder

It is extremely important to be aware of alternative causes of shoulder pain like inflammatory arthritis. Other important symptoms to be aware of are multiple joint pain/swelling, fever, altered sensation or altered sensation down the arms, fatigue, weight loss and night pain.

Again, frozen shoulder can be tricky to treat and is a notoriously long process, but treatment can assist. These may include:

Essentially, we aim to reduce pain and improve function in the fastest possible time. It is important to start treatment early, to improve mobility and reduce functional impairment. Where possible, we like to enforce conservative treatment and management. But in many cases, extra resources may be needed. In these cases anti-inflammatory injection therapy may be required. If patients are showing signs of advanced injury or disease then we may discuss the option for an MRI scanning, to get diagnostic clarity of what it is that may be causing the issue.

If a cause of your knee pain is suspected to be of an inflammatory or rheumatological origin, then you will be asked to provide a blood test and lab panel to check for raised inflammatory markers - ESR, CRP and TNFa, and maybe immunological makers.

Referral for Specialist Imaging or Private MRI Scans

At our musculoskeletal clinic we can provide ultrasound imaging to look for signs of tissue damage and inflammation. However, the shoulder joint often will require a more in-depth analysis using an MRI. If a private MRI scan is considered, we will put in a local referral to our Imaging Centre in Solihull. You will then be booked in within 5-10 days. Following your scan, the report will be completed by our colleague Dr David Beale (Consultant Musculoskeletal Radiologist). This report will then come back to the Dynamic Clinic and will we then discuss with you then best treatment plan. 

In cases where the disease progression is too severe, or the problem is not getting better, then we may refer to our trusted colleagues Mr Samir Massoud (Consultant Orthopaedic Surgeon) for a more invasive opinion.
