What are the causes of knee osteoarthritis?

Knee pain is the second biggest cause of musculoskeletal pain and disability, due to its huge complexity. With this osteoarthritis is the largest knee pain complaint. Osteoarthritis is caused from progressive degenerative changes of the joint cartilage. This often leads to signifiant pain, poor function and on going disability. Most cases of osteoarthritis and knee pain are triggered by a combination of age, postural mis-alignment, overuse, repetition, injury (or trauma), surgery, occupational strain, genetic predisposition and inflammation. Severity can vary, depending on cases of mild to moderate to severe.

Knee osteoarthritis can come in many different forms. Ultimately, what we want to reduce pain, improve function and slow the disease progression. At the Dynamic Clinic knee osteoarthritis is a very common issue we see many times every single day. Cases can range from mild-moderate osteoarthritis to severe. We always explain to every patient the importance of treating osteoarthritis early, to treat and slow degenerative changes. As osteoarthritis progresses, it can be notoriously difficult to treat. This is where surgery and joint replacement is often necessary, with total knee joint replacement being the most common orthopaedic surgery. That being said, we treat all cases of osteoarthritis.

Where possible, we like to enforce conservative treatment and management of knee pain and osteoarthritis. But in many cases, extra resources are needed and encouraged. We have built ourselves a very good reputation of delivering high quality regenerative treatments for knee pain. The most popular by far being P-PRP injection treatment, in which patients travel from all over the UK to see us for treatment (further information can be found on the ORTHOBIOLOGICS link below). If patients are showing signs of advanced injury or disease then we may discuss the option for an MRI scanning, to get diagnostic clarity of what it is that may be causing the issue.

If a cause of your knee pain is suspected to be of an inflammatory or rheumatological origin, then you will be asked to provide a blood test and lab panel to check for raised inflammatory markers - ESR, CRP and TNFa, and maybe immunological makers.

Referral for Specialist Imaging or Private MRI Scans

At the Dynamic Clinic we can provide ultrasound imaging to look for signs of tissue damage and inflammation. However, the knee joint can require a more in-depth analysis using an MRI. If a private MRI scan is considered, we will put in a local referral to our Imaging Centre in Solihull. You will then be booked in within 5-10 days. Following your scan, the report will be completed by our colleague Dr David Beale (Consultant Musculoskeletal Radiologist). This report will then come back to the Dynamic Clinic and will we then discuss with you then best course of treatment for your compliant.

In cases where the disease progression is too severe, or the problem is not getting better, then we may refer to our trusted colleagues Mr Jamie Arbuthnot (Consultant Orthopaedic Surgeon) or Prof Martin Snow (Consultant Orthopaedic Surgeon) for surgical opinion.

Knee Pain and Osteoarthritis Assessment

To help your knee pain and osteoarthritis, we firstly need to provide a functional knee assessment to check for severity. If you have been having knee pain which does not appear to be improving or is impacting on your function, it is recommended to see a musculoskeletal professional. This experienced clinician will be able to provide a full assessment, gain information to make a diagnosis and provide the best treatment and management strategies to resolve your pain and get you back to best function again. At the Dynamic Clinic, knee pain and injury is one of the most common issues we see many times every day. Our experienced musculoskeletal clinician, Mr Adam Whatley will be able to provide assessment, diagnosis and treatment of a wide range of knee pain conditions from sport injuries to osteoarthritis.

INJECTION TREATMENT FOR KNEE OSTEOARTHRITIS

Sometimes knee injuries to ligament, tendons, or cartilege can turn into chronic pain. In these cases, sometimes the body needs a push to help healing and repair. Injection therapy for knee pain, injury and osteoarthritis can also be commonly used to treat the stubborn condition and can deliver great success in many cases. There are many different forms of injection therapies but the most common are steroid injections, platelet rich plasma (PRP) injections and hyaluronic acid.

Steroid (cortisone) Injection Treatment for Knee Osteoarthritis

Individuals who have been suffering with knee pain who have not responded satisfactorily to conservative treatment and management may be advised to consider a cortisone injection to reduce pain and inflammation. However, it is very important to appreciate that steroid injections (if successful) are often only short-lived and may have an overall degenerative effect on the local tissue and cartilage if repeated. Although they can be good for inflammation, cortisone injections are not routinely used for knee pain management or knee tendons due to weakening of tissue. This can put tendons at an increased risk for rupture (tear), if repeated too often.

Platelet Rich Plasma (PRP) Injection Treatment for Knee Osteoarthritis

Hugely growing in popularity, platelet rich plasma (PRP) treatment is a safe natural treatment commonly used in many different cases of musculoskeletal pain and injury, that uses platelets from the patient’s blood to promote and facilitate healing and repair in damaged tissue. Increased scientific literature from evidence-based clinical trials are increasingly showing that platelet rich plasma (PRP) injection treatment is safe and effective for the treatment of knee pain, knee osteoarthritis and injury, also have improved benefits over cortisone, particularly for long term progress.
