WHAT IS GOUT?

Gout flare treatment Birmingham and Warwickshire

Gout is a type of inflammatory arthritis caused by a build-up of uric acid crystals within a joint. These crystals trigger sudden, severe attacks of pain, redness, swelling and warmth, commonly known as gout flares.

A flare may occur unexpectedly, often overnight, and typically peaks within hours. Without proper treatment, symptoms can last days or even weeks.

Gout is very common, affecting millions worldwide, and tends to occur in people who:

  • Have a family history of gout

  • Consume high-purine diets

  • Are overweight or have metabolic syndrome

  • Drink alcohol frequently

  • Have kidney problems or dehydration

  • Take medications that affect uric acid levels

What Causes Gout? (Understanding Uric Acid)

Uric acid is a natural by-product produced when the body breaks down substances called purines, found in certain foods and the body’s cells.

Gout develops when uric acid becomes too concentrated in the bloodstream (hyperuricaemia), leading to the formation of sharp urate crystals inside a joint.

Common Causes of raised Uric Acid include

  • High-purine foods (red meat, seafood, organ meats)

  • Alcohol (especially beer & spirits)

  • Sugary drinks (fructose)

  • Kidney impairment

  • Diuretics or blood pressure medications

  • Poor hydration

  • Genetic predisposition

When crystals form, they irritate the joint lining, causing extreme inflammation.

It is always important to be aware of more alternative factors of knee pain, like autoimmune related systemic inflammatory arthritis. Other important symptoms to be aware of are prolonged morning joint stiffness, increased swelling, multiple joint pain/swelling, fever, altered sensation or tingling down the leg, ongoing cramps in the legs,  fatigue, weight loss and night pain.

WHERE DOES GOUT COMMONLY OCCUR?

Although gout can appear in any joint, it most frequently affects:

1. Big Toe (1st MTP Joint)

The classic “gout toe,” presenting with intense pain and heat.

2. Ankle & Foot

Very common and often mistaken for sprains.

3. Knee

One of the most painful locations for gout; often treated with injection therapy.

4. Wrist, Fingers & Elbow

Seen in individuals with long-standing gout or high uric acid levels.

Typical symptoms include:

  • Severe, sudden joint pain

  • Swelling & heat

  • Red or shiny skin

  • Hypersensitivity to touch

  • Difficulty walking or moving the joint

WHEN SHOULD YOU SEE A MUSCULOSKELETAL SPECIALIST OR A RHEUMATOLOGIST?

It may be advised to seek specialist assistance and treatment if:

  • Pain is severe and limiting mobility

  • Oral medications (NSAIDs, colchicine) do not help

  • You have frequent gout flares

  • Swelling persists for several days

  • The joint is too painful to bear weight

  • You are unable to take standard anti-inflammatories

Prompt treatment prevents unnecessary suffering and reduces the risk of long-term joint damage.

Corticosteroid Injection for Gout – Fast & Effective Relief

Corticosteroid injections are common local treatment that work by directly targeting the inflamed joint, reducing:

  • Severe pain

  • Heat

  • Swelling

  • Redness

  • Joint stiffness

Benefits of corticosteroid injection for gout:

  • Relief often within 24–48 hours

  • Ideal when NSAIDs or colchicine are not suitable

  • Ultrasound guidance ensures accuracy

  • Highly effective for gout in larger joints (knee, ankle)

  • Allows quicker return to daily function

This is one of the fastest ways to calm a flare safely.

Chronic Gout Management & Prevention (Allopurinol)

Please be advised it is fundamentally important to understand that local joint injection treatment is for reducing pain and inflammation for acute flares and is NOT a treatment for management. To prevent future attacks, long-term management is essential.

This includes:

  • Allopurinol to reduce uric acid levels

  • Weight management

  • Reducing alcohol intake

  • Staying well hydrated

  • Reviewing medications with your GP

  • Monitoring uric acid through routine blood tests

  • Seeing a rheumatologist if things do not improve

Allopurinol is not used during a flare only as preventive therapy.
