What are the causes of sciatica?

Sciatica is a very common cause of back pain that is most often caused from impingement of an exiting spinal nerve root from spinal disc herniation. Sciatica is an umbrella term for pain along the route of the sciatic nerve as it travels from the lower back down the leg into the foot. There are many different causes of sciatica ranging from an acute cause of trauma or an accident to a slow progressive degenerative cause.

 

Sciatica is often caused from progressive degenerative changes to the spinal discs causing impingement on the nerve route. This often leads to signifiant pain, poor function and on going disability. Most pain will often start in the lower back and then refer in to the leg, often associated with altered sensation  (pins and needles) in the feet. Most cases of sciatica are triggered by a combination of overuse, repetition, injury (or trauma), occupational strain, genetic predisposition, age and inflammation. Severity can vary, depending on cases of mild to moderate to severe.

Symptoms of sciatica and back pain

It is extremely important to be aware of more sinister causes of back pain, like inflammatory arthritis, tumour or malignancy. Other important symptoms to be aware of are prolonged morning joint stiffness, fever, altered sensation in the saddle area, incontinence or tingling down both leg, ongoing cramps in the legs,  fatigue, weight loss and night pain. Cauda equina is a very serious condition that needs to be treated immediately in hospital. This is caused from a central disc herniation.

Sciatica can come in many forms. Ultimately, we want to achieve reduced pain, improved spinal function and prevent recurrence or chronicity. At the Dynamic Clinic, sciatica is an extremely common complaint we see many times everyday, in many different varieties. Cases can range from acute to chronic and mild to severe. We always explain to every patient the importance of treating sciatica early. As sciatica progresses, it can become increasingly difficult to treat. 

 

In the majority of cases, we like to enforce a conservative approach to sciatica treatment and management. But in certain cases, extra resources may needed. In these cases patients may be considered for the possibility spinal injection therapy. If patients are showing signs of advanced injury or disease then we may discuss the option for an MRI scanning, to get diagnostic clarity of what it is that may be causing the issue.

If a cause of sciatica or back pain is suspected to be of an inflammatory or rheumatological origin, then you will be asked to provide a blood test and lab panel to check for raised inflammatory markers - ESR, CRP and TNFa, and maybe immunological makers.

Referral for Specialist Imaging or Private MRI Scans

At the Dynamic Clinic we can provide ultrasound imaging to look for signs of tissue damage and inflammation. However, the spine will require a more in-depth analysis using an MRI. If a private MRI scan is considered, we will put in a local referral to our Imaging Centre in Solihull. You will then be booked in within 5-10 days. Following your scan, the report will be completed by our colleague Dr David Beale (Consultant Neuro-radiologist). This report will then come back to the Dynamic Clinic and will we then discuss with you then best course of treatment for your compliant. 

In cases where the condition is too severe, or there is suspected neurological compromise then we may refer to our trusted colleague Mr Spencer Harland (Consultant Spinal Neurosurgeon) for surgical opinion. In occasions where the disc may be causing significant nerve compression, a micro-discectomy may be used to take part of the disc or provide spinal fusion.

SPINAL INJECTION TREATMENT 

Sometimes sciatica can turn into chronic pain. In these cases, sometimes the body may need extra assistance to control pain, help healing and repair. Injection therapy for sciatica can also be commonly used to treat the stubborn cases that have not responded well to conservative measures. There are many different forms of injection therapies but the most common are steroid injections, platelet rich plasma (PRP) injections and prolotherapy.
