What are the causes of back pain?

​​Back pain is one of the most common causes of pain and disability worldwide, and can range from short episodes of localised discomfort through to extremely significant levels of pain. Most cases of back pain, especially low back pain, are triggered by a combination of, poor posture, overuse, repetitive strain, injury (or trauma), and inflammation to the structures associated with the spine. Severity can vary, but below are some examples of common causes of back pain.

What are the common types of back pain?

  • Soft tissue injury to muscles, tendons, ligaments

  • Spinal facet joints inflammation 

  • Injury or irritation to spinal nerves that pass through the spine 

  • Degenerative changes to spinal joints

  • Compression or reduced blood flow of the spinal nerves or spinal cord

  • Disc injury such as bulging or herniated disc that can compress nerve roots (sciatica)

  • Degenerative changes to disc

  • Spinal arthritis

  • Osteoporosis 

 

It is also very important to be aware of more sinister causes of back pain, like inflammatory arthritis, or tumours. Other important symptoms to be aware of are prolonged morning joint stiffness, fever, altered sensation or tingling down both legs and/or around the genitals/anus, ongoing cramps in the legs, abdominal pains,  bowel and bladder incontinence, fatigue, weight loss and night pain.

Back pain and injury is one of the biggest complaints we see here at the Dynamic Clinic, seeing multiple times every day. Fortunately, the vast majority of back pain cases we see are dealt with and treated successfully with conservative treatment modalities. We are very proud of the fact that less that 5% of our back pain patients are considered for invasive therapeutic treatment. If patients do not respond adequately to conservative treatment or the problem is getting worse, then consideration for injection therapy may be discussed. If patients are showing signs of neurological deficit then we may discuss the option for an MRI scanning, to get diagnostic clarity of what it is that may be causing the issue. 

​

If an inflammatory or rheumatological cause of your back pain is suspected, then you will be asked to provide a lab panel and blood test to check for raised inflammatory markers - ESR, CRP and TNFa, and maybe immunological makers.

Referral for Specialist Imaging and Private MRI Scans

At the Dynamic Clinic we can provide ultrasound imaging to assess for signs of tissue damage and inflammation. But, occasionally, we may require a more in depth analysis. When a private MRI is considered, we will put in a local referral to our close Imaging Centre. You will then be contacted and booked in within 5-10 days. Following this scan, the report will be completed by our colleague Dr David Beale (Consultant Neuro-radiologist). This report will then come back to us and will we then discuss with you then best course of action. 

In some cases we may refer to our trusted colleagues Dr Abir Dodger (Consultant Anaesthetist) or Mr Spencer Harland (Consultant Spinal Neurosurgeon) for opinion on spinal denervation or for surgical option on micro-discectomy.

Back Pain Assessment

To help your back pain, we firstly need to know what is going on. To do this we have to provide a thorough back pain assessment.​​ If you have back pain neck pain which does not appear to be getting better and impacting on your function, it would be recommended to see a musculoskeletal professional. Mr Adam Whatley, our clinic director and musculoskeletal specialist will be able to provide a full assessment, gather information to make a diagnosis and provide the best treatment and management strategies to resolve your pain or injury and get you back to full function again.
